Assessment and Action Records

While you are looked after your social worker will have to complete an “Assessment and Action Record”.  They will need your help to do this and may ask other people that help look after you, like your carers or teachers, to help fill it in too.

The form helps people looking after you to think about how well you have done each year and what they need to do to look after you properly.  So, they look at different parts of your life, like education, health, your physical growth and how you are emotionally.  They see what changes there have been in a year and what needs to be done to make sure that you continue to grow and develop well and happily.

The form will be kept on your file and in years to come you can see if they did everything that they said they would!
