Care Plan
A “care plan” can be many things. There are formal care plans which are submitted to court if Children, Schools and Families is requesting a “care order”. This lays out the plans for the sort of placement they are planning for you and details things such as where you would go to school, plans for you to see family members and any important health plans for you.
When you first come into care your social worker writes a care plan as part of the planning about who you will live with and so on. There will always be a written care plan that lays out how CSF are planning your care. In a children’s home you will also have a care plan that the staff who are looking after you will have written with your help. It’s an important record that lets everyone know information about looking after you from the foods you don’t like to eat, any medicines you have to take, to how you like to be woken up in the morning.
Your care plan will be updated regularly as you grow up and your tastes, needs or circumstances change. Your statutory review (this is a review that care workers must do) is a chance for those involved in caring for you to look at your care plan and see what needs to be changed or stay the same. You will be asked for your thoughts on your care plan too.
If you disagree with anything in your Care Plan you should tell your social worker.
